Overflow Student Ministry Permission Form

By completing this form, I give permission for my student to participate in all Locust Hill Baptist Church sponsored events. In case of emergency, I give Locust Hill Baptist Church staff member or appointed chaperone permission to treat my child. I will not hold the individual, the attending physician, Locust Hill Baptist Church, or any agent thereof responsible. My child and I understand what is expected regarding behavior. Bad behavior will result in immediate consequences, forfeiting the privilege of participation in the next event and being sent home from the event at the parent or guardian’s expense. I understand there will be times when cell phones cannot be used and may be confiscated if misused. Furthermore, no other electronic devices or gaming consoles are permitted unless given permission by LHBC staff. NO drugs, alcohol, or weapons of any kind will be allowed.

I, the parent or guardian of the above child, acknowledge the participation in youth activities sometimes necessarily involves risk of physical injury. I further acknowledge that Locust Hill Baptist Church Student Ministry programs are primarily administered by a volunteer, a person who gives their time rather than a paid professional. By signing this form on behalf of the above-named child and permitting the voluntary participation of said child is in its Student Ministry programs, I hereby release, discharge, and hold harmless Locust Hill Baptist Church, its employees, volunteers, agents, and other representative from any claims arising out of or relating to any physical injury that may result to said child while participating in Locust Hill Baptist Church Student Ministry sponsored activities. I further understand that pictures, videos, or audio may be used at the discretion of Locust Hill Baptist Church without notification.
